What Is Interoception?
Interoception is the sense of the internal state of your body. It is how your brain perceives signals from your organs — your heart, lungs, stomach, bladder, skin, and viscera — and translates them into conscious feelings like hunger, thirst, the urge to breathe, or the sensation of a racing heart. It is often called the "eighth sensory system," and it is arguably the most important one for understanding your emotional life.
The term was first defined by the physiologist Charles Sherrington in 1906, but it has only been studied intensively in the last two decades. In thirty-one years of archiving, I learned that the most reliable intelligence did not come from documents alone. It came from body awareness — the ability to read the subtle physiological signals that reveal whether someone is telling the truth, hiding something, or about to break. Interoception is how you read your own signals. It is the foundation of self-intelligence.
The Difference Between Interoception, Proprioception, and Exteroception
To understand interoception, you need to distinguish it from the other two body-sensing systems:
- Exteroception: The five traditional senses — sight, hearing, touch, taste, smell. These detect stimuli from outside the body. They tell you about the world.
- Proprioception: The sense of where your body is in space. It tells you the position of your limbs, your posture, your movement. It is how you can touch your nose with your eyes closed.
- Interoception: The sense of your body's internal state. It tells you about your heart rate, breathing rate, temperature, digestion, and visceral sensations. It is how you feel your feelings — literally.
Most people confuse proprioception and interoception, but the distinction is critical. Proprioception tells you where your arm is. Interoception tells you how your arm feels. One is about position. How Interoception Shapes Emotional Experience
This is the most important thing to understand about interoception: emotions are not abstract mental events. They are your brain's interpretation of interoceptive signals. When your heart beats faster, your brain notices the interoceptive signal and assigns an emotional label based on context. If you are in a dangerous situation, that is fear. If you are about to give a speech, that is nervousness. If you just saw someone you love, that is excitement.
The same physical signal — accelerated heart rate — can produce completely different emotions depending on how your brain contextualizes the interoceptive data. This is called the James-Lange theory of emotion, and modern neuroscience has confirmed its core insight: you feel your body, and then your brain constructs an emotional narrative to explain what you felt.Barrett LF. This means your interoceptive accuracy directly determines the quality of your emotional life. If you can read your body's signals precisely, you can regulate your emotions. If you cannot — if your interoception is poor — you will experience emotions as overwhelming, confusing, or disconnected from any identifiable cause.
Poor Interoception: The Hidden Driver of Anxiety
Poor interoception manifests in two opposing patterns, both of which fuel anxiety. The first is interoceptive hypersensitivity: you are too aware of body sensations. You notice every heartbeat, every breath change, every digestive rumble, and you interpret these normal signals as signs of danger. A slight increase in heart rate becomes "I am having a heart attack." A shallow breath becomes "I am suffocating." This is the biological pattern underlying panic disorder.Domschke K, et al. (2017). The second pattern is interoceptive hyposensitivity: you are not aware enough of body sensations. You miss the early signals of stress — the subtle tension in your shoulders, the slight quickening of your breath — until the anxiety has already peaked. You feel like anxiety comes out of nowhere because your interoception failed to detect its gradual buildup. This pattern is associated with generalized anxiety disorder and alexithymia (difficulty identifying emotions).
Both patterns represent a breakdown in the feedback loop between body and brain. The body sends signals. The brain either amplifies them into catastrophe or ignores them until they become overwhelming. Either way, the result is dysregulation.
Interoception and Trauma: Why Body Awareness Matters
Trauma has a profound effect on interoception. Many trauma survivors develop what is called interoceptive avoidance — they learn to disconnect from body sensations because those sensations are associated with the traumatic experience. The body becomes dangerous. Ignoring it becomes a survival strategy.
This is the biological basis of dissociation. When you disconnect from interoception, you disconnect from your emotions, your needs, your intuition, and your ability to regulate. Recovery from trauma requires rebuilding the interoceptive connection. Not by forcing yourself to feel everything at once — that can be re-traumatizing — but by gradually, gently re-establishing the brain-body dialogue through structured practices. This is what somatic exercises are designed to do.
The Vagus Nerve as an Interoceptive Highway
The vagus nerve is the primary physical infrastructure for interoception. Approximately 80% of vagal nerve fibers are afferent — they carry sensory information from the body to the brain, not the other way around. This means the vagus nerve is primarily a data cable. It continuously reports on the state of your heart, lungs, digestive tract, and immune system to the brainstem nuclei that process interoceptive information.
The nucleus tractus solitarius (NTS) in the brainstem is the main processing hub for vagal interoceptive data. From there, signals are relayed to the insula — the brain region that generates conscious interoceptive awareness — and the anterior cingulate cortex, which assigns motivational and emotional significance. When vagal tone is high, this interoceptive data stream is clear and accurate. When vagal tone is low, the signal is noisy, distorted, or suppressed.Craig AD. (2015). How Do You Feel? This is why vagal toning improves emotional regulation: it improves the quality of the interoceptive data your brain receives. Clearer data means more accurate emotions. More accurate emotions means better regulation.
How to Measure Interoceptive Accuracy
Researchers measure interoceptive accuracy using standardized tests. The most common is the heartbeat detection task (also called the heartbeat counting task). You are asked to silently count your heartbeats during a timed interval — 30, 45, or 60 seconds — without taking your pulse or using any external aid. Your counted number is compared to an actual ECG reading. The closer your count, the higher your interoceptive accuracy.
Scores on this test vary widely. Some people are within 10% of their actual heart rate. Others are off by 50% or more. Studies consistently find that lower interoceptive accuracy correlates with higher anxiety, more panic attacks, greater emotional dysregulation, and poorer decision-making.Garfinkel SN, Critchley HD. The good news is that interoceptive accuracy is trainable. It is not a fixed trait. You can improve it with practice, which is exactly what the exercises below are designed to do.
Exercises to Improve Interoception
Improving interoception requires consistent practice in attending to body sensations. Here are the most effective exercises:
- The body scan: Lie down or sit comfortably. Systematically bring your attention to each part of your body — feet, legs, torso, arms, neck, head. Notice whatever sensations are present without trying to change them. Do this for 10 minutes daily.
- Heartbeat awareness: Sit quietly and try to feel your heartbeat without touching your chest or wrist. Do not count. Just notice. Do this for 3 minutes, twice a day. This directly trains the interoceptive pathway.
- Somatic tracking: Choose one body sensation — the weight of your body in a chair, the feeling of your breath at your nostrils, the temperature of your hands — and hold your attention on it for 30-60 seconds. When your mind wanders, gently bring it back.
- Vagal breathing with attention: Practice 4-6 breathing (inhale 4, exhale 6) while paying close attention to the physical sensations of the breath cycle — the movement of your chest, the temperature of the air, the pause between breaths. This combines vagal toning with interoceptive training.
- Hunger and fullness awareness: Before eating, notice the physical sensations of hunger. After eating, notice the physical sensations of fullness. This trains interoception around a natural, accessible body signal.
Somatic Practices and Interoceptive Training
Somatic practices — body-based therapeutic approaches — are interoceptive training by another name. Approaches like Sensorimotor Psychotherapy, Somatic Experiencing, and the somatic therapy exercises I have documented elsewhere all work by improving interoceptive accuracy. They teach you to notice body sensations, tolerate them without reacting, and use them as data for emotional regulation.
The key principle is pendulation: gently moving attention between a sensation of discomfort and a sensation of safety or neutrality. This teaches the nervous system that body sensations are not threats. They are signals. You can notice them without being controlled by them.
