1. Diagnostic Criteria: The 600 pg/mL Norepinephrine Threshold

While Postural Orthostatic Tachycardia Syndrome is often generalized as a single condition, autonomic cardiology recognizes three primary pathophysiological subtypes: neuropathic, hypovolemic, and hyperadrenergic POTS. Affecting approximately 30% to 50% of the POTS population, the hyperadrenergic phenotype is characterized by a massive neurochemical surge upon assuming an upright posture.

The formal clinical diagnosis requires two specific objective criteria:

  1. Biochemical Surge: Standing plasma norepinephrine (noradrenaline) ≥ 600 pg/mL (3.55 nmol/L) measured after 10 minutes of active standing or 70-degree head-up tilt. Normal healthy individuals display standing norepinephrine between 250 and 450 pg/mL.
  2. Hemodynamic Pattern: Sustained increase in systolic blood pressure of ≥10 mmHg (often jumping by 20 to 40 mmHg) concurrently with the diagnostic ≥30 bpm heart rate increase upon standing—a phenomenon known as orthostatic hypertension.

2. Central Sympathoexcitation vs. NET Transporter Dysfunction

Two distinct molecular mechanisms drive this extreme sympathetic surge:

  • Central Sympathetic Hyperactivity: The rostral ventrolateral medulla (RVLM) in the brainstem fires uninhibited sympathetic bursts down the intermediolateral cell column of the spinal cord, overwhelming parasympathetic vagal counter-regulation.
  • Norepinephrine Transporter (NET) Deficiency: In 2000, Shannon et al. identified a missense mutation (Ala457Pro) in the SLC6A2 gene encoding the presynaptic Norepinephrine Transporter (NET). 4. Targeted Pharmacotherapy: Clonidine, Guanfacine & Combined Blockade

Standard POTS treatments like midodrine (an alpha-1 agonist) are strictly contraindicated in hyperadrenergic POTS because vascular beds are already excessively constricted. Therapeutic management requires central sympathetic suppression:

  • Central Alpha-2 Agonists (Clonidine & Guanfacine): Stimulate presynaptic α2-adrenergic receptors in the brainstem RVLM, functioning as a negative-feedback shutoff valve that halts central norepinephrine discharge. Transdermal clonidine patches provide steady, round-the-clock sympathetic suppression without peak-and-trough rebound spikes.
  • Methyldopa: A centrally acting false neurotransmitter converted to alpha-methylnorepinephrine, dampening central sympathetic tone with a longer half-life than oral clonidine.
  • Dual Alpha/Beta Blockers (Labetalol, Carvedilol): By blocking both β1 receptors (slowing heart rate) and α1 receptors (preventing excessive vasoconstriction), these agents control both tachycardia and orthostatic hypertension without unopposed vasoconstriction.

5. Diagnostic Differential: Hyperadrenergic POTS vs. Pheochromocytoma

Because standing norepinephrine levels can soar above 1,500 pg/mL, ruling out a pheochromocytoma (a catecholamine-secreting tumor of the adrenal medulla) or paraganglioma is mandatory. In pheochromocytoma, catecholamines (particularly metanephrine and normetanephrine) are elevated in 24-hour urine and plasma regardless of posture, accompanied by paroxysmal hypertensive crises. In hyperadrenergic POTS, supine catecholamines are typically normal and spike exclusively upon assuming an upright stance.
